In the case of an application from the individual, we will confirm the identity through verification of identification documents such as a driver's license, passport, health insurance certificate, or seal certificate, a callback to the phone number registered with us, and confirmation of our registered information such as name, address, phone number, PIN, transfer account number, etc.
For applications from a representative, we verify their status as a representative through a power of attorney document and the seal certificate with the imprinted seal on the document, along with potential contact with the individual.
(2) Fees
If we provide a response through written documents for requests such as disclosure, a fee of 500 yen per request will be charged, and payment should be made in cash or other specified methods.
